INCEPTION:
The idea of TPA was formulated in early 1994 when it became apparent that many physicians were leaving private practice to participate in managed care. Since the basic IPA structure includes a NETWORK, it was becoming increasing difficult to direct private pay patients to independent offices. Although many of us are active in the medical community, there is no easy way to discern another physician's contractual arrangements. With every lost private pay patient and independent office, the probability of a complete collapse of TRADITIONAL medicine increases. Thus, TPA was formed to provide independent physicians a stable referral base and help preserve patient choice.


MISSION STATEMENT:
Traditional Practice Alliance will attempt to sustain TRADITIONAL MEDICINE and INDEPENDENT PHYSICIAN PRACTICE through networking. The emphasis will be focused on affordable, direct patient care instead of cost containment. The Network is not designed to disrupt established referral patterns. TPA physicians will only aid those patients requesting a more traditional medical office. All referrals will be made in the best interests of the patients.

STRUCTURE:
TPA will work initially simply as a referral base. Once the Network is functioning, the physician panel will be marketed to various PPOs and indemnity plans. The organization is structured as a nonprofit medical care foundation. There is no financial control over the independent offices. TPA will act as a messenger to its panel members in presenting insurance information. This allows for office autonomy and avoids the high administrative costs often associated with IPAs.


PHYSICIAN PANEL:
The Sacramento medical landscape has a replete supply of various IPAs and medical groups. All have been structured to avoid cost with little attention to quality. Thus, for yet another organization to form, it must distinguish itself and stand alone. Because of this, and the desire to address the patients' needs first, we are attempting to form a panel of physicians who strongly believe in a traditional practice format. The physician participants must be board certified, have a good malpractice history, and provide direct care to their patients. They must be engaged in the private and independent practice of medicine, not subject to the direction or control of any other person or entity.


SUMMARY:
It is the structure of TPA that will allow it to survive in an HMO dominated world. We will be able to provide less costly, quality medicine for our patients. The cost savings will come from a marked decrease in administrative costs, lower over utilization and less malpractice. The companies we service will truly be getting the best of traditional medicine.
